Social Workers

Whether it’s helping a new resident transition to our home or leading a group therapy session for residents’ families, our licensed social workers strive to meet the varied social and emotional needs at our home. They are an integral part of the care team that helps residents to maintain optimum health. They also are a resource for families wishing to know more about their loved one’s unique needs.

Pre-Admission Screening and Care Plans

Our social workers provide the necessary support for families and residents who have decided to move their loved one to Seashore Gardens Living Center. They perform an initial intake assessment to gather data to help with developing the individualized patient care plan. Then, they continue to make periodic assessments of residents to identify and address any changing social and emotional needs.

Navigating the Continuum of Care

It’s a life changing decision to move from one’s own house to Seashore Gardens Living Center. Similarly, it is an emotional one to move down the continuum of care, as needs change. Our in-house social workers help residents transition as needed from independent living to skilled nursing care. They use a biopsychosocial approach which considers biological, psychological, social, and spiritual factors to guide residents in determining the best living arrangements. Because families are an integral part of the decision making process, our social workers are there every step of the way for a resident’s loved ones, as well.

Alzheimer’s Support Group

Family members and caregivers of people with Alzheimer’s or Dementia are invited to a free Alzheimer’s Support Group meeting at Seashore Gardens Living Center on the third Wednesday of every month at 4:30 pm. The meeting is led by a certified social worker. This is an opportunity to learn about resources, find strategies for dealing with a loved one with memory loss, and meet others in similar circumstances.
